The following are examples of information that is needed to complete the application:

1. Education History - School names, complete addresses and dates of attendance.

2. Employment History - Complete addresses, phone numbers and dates of employment.

3. Residence History - Complete addresses of the locations that you have resided in the past 10 years.

4. Military History - If any, unit names, locations, and contact phone numbers.

5. Personal References and Acquaintances - Provide names, complete addresses and phone numbers of personal references you have known for the past 5 years.
